Hazel Arlene Hazelwood, 68, of Keavy, passed away on Saturday June 18, 2016 at the UK Medical Center in Lexington, KY.
Funeral service for Hazel Hazelwood will be held at 1 p.m. on Tuesday June 21, at the Vankirk-Grisell Funeral Home with Rev. Dennis McCoy and Rev. Shane Martin officiating.
Burial will follow in Locust Grove Cemetery in Keavy.
